Since April 2026 my Honda Crv 2021 had been having issues with all the warning lights on my dashboard turning on and off. When all the lights turn on the sensors don’t function the way they supposed to and won’t alert me when I’m getting close to a car or need to make a sudden break. I took it to Autozone to get the codes from the warning lights and also to Honda and both said that I needed to replace, the fuel injectors. I replaced the fuel injectors and the problem still there. It was inspected by Autozone, Honda and also an independent auto shop.

FORWARD COLLISION AVOIDANCE

I was approaching an exit on the highway going about 60 mph when my car braked suddenly causing rapid deceleration. I was not touching the brake pedal and in fact tried to push the accelerator to no effect. Only lasted a few seconds but very frightening as there were cars behind me also driving at that speed.

In the past two weeks, the automatic brake light came on when there was nothing in front of me. Then this past Thursday, I was in the middle lane approaching a stoplight that was about 500 ft. ahead of me, nothing was in front of me and the brake light came on and the car made a quick and hard stop. If anyone was behind me, they would not have been able to stop. Now I am very leery of driving my car, and I disable this feature so I feel safe transporting myself and my family.

FORWARD COLLISION AVOIDANCE · STEERING

The steering wheel is slightly sticking while driving/steering on interstates and highways where mostly noticed, but also noticing on curved backroads. When navigating a curve the steering wheel has slightly stuck and the automatic brake light flashes on very briefly. This has happened several times now and both main drivers of the car have noticed the issue. We are reporting now out of fear that it may become a bigger issue if not addressed, that affects both steering and braking that puts occupants at risk. We are reporting this as of today, which is the most recent incident on May 25, 2026, but this has happens about 4 or 5 times in the last month. We checked to see if this was a known recall issue, but the recall only applies to 2023 - 2025 CRVs for some reason. This sounds like the exact issue/recall that our 2021 crv is having. Thank you!

FORWARD COLLISION AVOIDANCE

While driving at 60 mph, automatic braking feature activated slamming on brakes unexpectedly. There was no car in front of me when this occurred. This has occurred several times. In this instance, we were driving on a 4 lane HWY in the right lane. A car passed me in the left lane and just after passing me the braking system slammed on the brakes. The other vehicle was still in the left lane while we were in the right lane.

Additionally, while driving with the cruise control set at 72 MPH, the vehicle unexpectedly decelerated to 66 MPH before accelerating to 73 MPH. No warning light was illuminated. The vehicle was taken to the dealer, but the cause of the failure could not be determined. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 81,000.

I was driving past a school zone at 20 mph, came to a stop to allow children to cross street, as I began my acceleration past the school crossing, I reached a 20mph speed again and my CRV activated the emergency braking system without having anything in front of me that could have activated the forward collision braking system. Luckily I was not rear ended.

CRV Dashboard all warning lights illuminated including check engine and emissions system problem. I had to park my car and it smelled like gas. I immediately called Honda Care and I took my car to Unicars Dealer on 03/11/2026 They provided a diagnostic report, report retrieved PO172 fuel system Bank 1 too rich and P219F no 4 cyclinder air fuel ratio variation. Despite Honda issuing Service Bulletin NHTDA ID 10220352, 10208090 and similar 22-002 for the exact same part failure 2017-2019 models, and despite Manufacturer Communication MC -10201454 specifically requesting failed injectors from 2021 models for investigation, HONDA HAS REFUSED TO PROVIDE A WARRANTY EXTENSION OR GOODWILL ASSISTANCE FOR THIS KNOWN DEFECT. This is a premature failure of critical fuel system component that causes the vehicle to stall smell like gas, lose power unexpectedly posing a significant safety risk to the driver and others on the road. I opened a GoodWill case # 16219868- after two weeks Case manager said it did not meet their guidelines offered a $125 assistance. I have called Unicars dealer service technician multiple times with no avail and no return calls. I finally walked into the UNICARS Dealer yesterday 3/25/26 and made appointment to get vehicle fixed on 03/27/26. I am paying out of pocket for all repairs over $2000 dollars.

Dear NHTSA, There needs to be an immediate recall on this (VIN#: [XXX] ) 2021 Honda CR-V Hybrid Motor Nightmare. Despite proper maintenance and care, the vehicle has experienced persistent problems that stem from known defects. The engine has a fatal flaw with the head gasket. The high compression turbo engine stretches the head bolts, lifts the cylinder head just enough to blow the gasket causing coolant to leak into the cylinders and mix with the oil and destroys the bearings. Along with oil dilution where gas washes pass the piston rings. This is a widespread problem affecting this model as indicated by the numerous technical service bulletins. The issue appears to occur during the 20,000 to 30,000 mile mark where it begins to experienced recurring engine performance issues, misfires, loss of power, and excessive radiator fluid consumption resulting in premature engine wear and sludge buildup. Honda sites have pointed out: "This can happen when the car is frequently idled or driven for short commutes, which can prevent the oil from flowing properly" and my commute is a 3 minute drive and 5 minute to the market. [XXX] "it’s a well documented issue and Honda America is aware of it that they’ve released an article about it for mechanics" One customer wrote "I have a 2021 Honda CRV and the engine had a terrible knock" [XXX] Please do something immediately! Several Mechanics acknowledge these widespread issues, the Lane Manager that advised me to reach out to the manufacturer pointed out that this is a “common issue”, and Your Website highlights that you all have already received similar recall campaigns for the engine oil and gasoline dilution issue that occurs approximate at the 22,500 mileage. My dash board lit up and started dinging. Many different messages were flashing across the screen such as driver assist feature problem, TPMS problem, and others related to the electronic driver assist features. I noticed a difference in the way my car handled as I was pulling off the highway to a gas station to check my tire pressure. It seems drivable at the moment and I don't have another vehicle option. It's a 2021 CRV.

The contact stated that while parking, and the brake pedal was depressed, the vehicle accelerated inadvertently up a slight incline and collided with a stone boulder/wall. Neither the contact nor the passenger sustained injuries. The contact stated that no collision warnings were activated to detect the collision. The air bags did not deploy. The vehicle was towed to a dealer, where the failure could not able to be replicated. The vehicle was then towed to an independent mechanic, where the vehicle has yet to be diagnosed. The manufacturer was not notified of the failure. The failure mileage was approximately 56,000.

FORWARD COLLISION AVOIDANCE

About a year ago, I was driving at speeds of approximately 10-15 mph when my car braked and immediately brought me to a dead stop despite no obstacles being present in the roadway. It appears the automatic emergency braking system malfunctioned. Had there been a car behind me, it likely would have caused an accident. On August 15, 2025, I was driving on the freeway using the adaptive cruise control when traffic in front of me slowed, engaging the automatic emergency braking. Traffic in front of me cleared almost immediately but my car was still very rapidly decelerating. I pressed the gas pedal, but the vehicle continued to bring me to almost a complete, immediate stop on the freeway (I had been traveling at 70-80 mph prior to the stop). I feel incredibly fortunate that I did not end up in an accident due to the traffic behind me and that no one was injured or killed. I pulled off onto the shoulder when it became clear that I was not going to be able to quickly regain control of my vehicle. Driving a vehicle without being able to properly control it was an absolutely terrifying experience. I was bracing myself for impact from behind as I desperately tried to regain control and accelerate back up to the flow of traffic. I am certain that this malfunction will result in deaths if not addressed. On August 23rd, 2025, I was driving on a highway at approximately 60 mph when the car in front of me slowed in preparation for a right turn off the road. I had disabled the CMBS feature at the beginning of my drive, yet the car still braked without me hitting the brake pedal. It was not an immediate, abrupt brake as I had experienced in the other two previously described incidents, but it was still highly unnerving to feel another loss of control of the vehicle, especially since the system was supposedly disabled.

• During the short time I have owned this vehicle the automatic emergency braking system has engaged 3-4 times when there was no object in front of the vehicle. The vehicle can be made available upon request. • All incidents happened when driving on the freeway. Fortunately no one was following me which could have resulted in my being rear ended for a sudden stop. • I took the vehicle into the dealership service center today (8/8/25) where they told me it is normal and I should just be a good driver, keeping my distance from vehicle to the front of me. They stated that if they were to replace it the same thing would occur. The problem has not been reproduced by the service center. • When these issues have occurred on one occasion the braking alert sounded ( beeping noise) and then the car started braking. On the other occurrences I do not recall the alert. The car just started braking. Other than that there were no warnings it was going to happen.

I am reporting a serious safety issue with my 2021 Honda CR-V involving the Collision Mitigation Braking System (CMBS). On multiple occasions, the vehicle has applied the brakes suddenly and forcefully without any obstacle or vehicle in front of me. THIS HAD HAPPENED OVER 20 times since i bought the CAR on March 10 2024. The most recent incident occurred while I was driving at highway speed on a clear, dry day. The CMBS system engaged abruptly, causing the car to decelerate rapidly and SLAMED BREAKS automatically. There were no vehicles, pedestrians, or obstructions ahead. This unexpected braking could have easily resulted in a rear-end collision, endangering me and other drivers. I have kids in the back sit and the car behind us almost hit my car. I am afraid the result might have been much worse if there was TRUCK behind my car. I took the vehicle to the Honda dealership, but they were unable to replicate the issue and did not offer a fix. They also mentioned that unless the problem could be duplicated, any diagnostic or repair fees would not be covered, despite this being a known issue under investigation by the NHTSA. I am aware that the NHTSA has an open Engineering Analysis (EA24-002) into this very issue affecting millions of Honda vehicles. I’m deeply concerned that this problem continues to put drivers and passengers at risk and believe a formal recall is necessary. I request that this issue be escalated and that Honda be held accountable for a fix before someone gets seriously injured or killed.
